Clutter and food leftovers were 'visible throughout' a centre for people with disabilities in Waterford, according to the health regulator.
Meanwhile two HSE-run centres were advised to improve their services, with one HIQA report highlighting shortcomings in staff and the general welfare of patients at a facility in Donegal.

In total, 20 of the 32 centres inspected by HIQA were found to be non-compliant.
